I hereby state that I am in good health and I know of no medical reasons as to why I cannot take part in physical activity in Gymco.

I agree to comply with all rules imposed by the company regarding the use of the facilities and equipment. I agree to conduct myself in a controlled and reasonable manner at all times, and to refrain from using any equipment in a manner inconsistent with its intended design and purpose.

I assume full responsibility for any and all injuries, losses & damages that I incur while attending, exercising or participating at Gymco. I hearby waiver all claims against Gymco, its instructors,or partners of individually or otherwise, for any injuries, claims or damages that might incur.
